Please allow about an hour for your initial office visit that will include a comprehensive evaluation that will lay the groundwork for finding and treating the cause of your medical condition.
Please download, print and complete the four “New Patient Forms” listed below, and bring them with you to your first appointment.

Also, in addition to bringing all the new patients forms with you to your first office visit, please bring the following with you:
- All supplements (vitamins, minerals, herbs, etc.) that you are currently taking or have taken in the past three months.
- A list of all prescription medications and/or over-the-counter medication that you are currently taking. This includes laxatives, nasal sprays, birth control, etc.
- Any laboratory or diagnostic test results that you have from the last 2 years.

FRAGRANCE-FREE POLICY: Due to the sensitivity of some of our patients, we ask that all visitors to our office please avoid using or wearing scented products such as perfumes, colognes, lotions, fragrances or essential oils. The fumes from these products may cause some of our patients to suffer adverse reactions. We thank you for your consideration of others and your cooperation with this policy.
PAYMENT POLICY & INSURANCE: We are a private practice and do not accept assignment from any insurance company. Payments are due at time of service and we accept payment by cash, personal check, VISA, MasterCard, American Express and Discover.
If you have PPO Insurance, payments are due at the time of service and we will provide a computerized bill with appropriate medical coding that can be sent to your PPO insurance carrier for reimbursement on covered medical services. Office visits are paid in full by the patient. Standard laboratory tests are billed by the performing laboratory directly to your PPO insurance company. Specialty tests are to be paid by the patient at time of service.
We do not accept HMO Insurance, and are not Medicare or MediCal providers.
