Is It Sunshine or Vitamin D That Reduces Your Risk To Chronic Diseases? An Interview with Karl J. Neeser, PhD

Dr. Karl J. Neeser is the co-author of the book, “The Key to Health and Longevity” (with Ratana Somrongthong, PhD). During his research of the book he became aware of the inverse association between vitamin D and many non-communicable (non-transmissible, chronic, lifestyle) diseases. Vitamin D is really a hormone with receptors on virtually every tissue in the body. Since we have evolved as a species being exposed to the sun, the dramatic reduction in sun exposure from fear of sun, in addition to greater amounts of time indoors due to one’s occupation and general living conditions has put us at risk for vitamin D deficiency and deficiency of whatever physiologic benefits come from sun exposure.

Risk factors to these non-transmissible diseases include reduced physical activity, vitamin D insufficiency, reduce quality of sleep and poor diet.

Vitamin D absorption from the diet is reduced as we age. Fifteen to 20 minutes in the mid-day sun is optimal for manufacturing adequate vitamin D levels. This is “non-burn” tanning of the skin. As soon as there is a slight reddening or “pinking” of the skin one should cover up and get out of the sun.

Vitamin D in the blood should be at least 50 ng/ml. Oral doses of vitamin D3 may be needed to get above 50 ng/ml.  Several thousand IU of vitamin D may be needed to achieve this level. It is probably better to get the vitamin D from the sunshine than a pill. There appears to be additional benefits to sunlight exposure that go beyond the increase in vitamin D levels.

Supplementing with high doses of vitamin D may suppress endogenous production of this hormone which may have adverse consequences.

The central nervous system (CNS), eyes and cardiovascular system all have vitamin D receptors. While sun exposure has been associated with certain skin cancers it is protective against “black” melanoma, the most malignant form.

It is noted that some Asians have a preference for light skin and avoid the sun intentionally. Intravenous vitamin C and glutathione have been used to lighten their skin.

There are many diverse dietary beliefs for longevity and optimal health. General recommendations are to not eat sugar; eat good fats; fast for at least fourteen hours daily; eat an early dinner (5-5:30); eat vegetables, fruit and fish.

Reducing EMFs, getting sunlight, exercise and get good sleep are longevity fundamentals. Self-responsibility is a must for a healthful lifestyle and longevity.

Learning new Salsa footwork, something I am not good at, I challenge myself to learn on a regular basis. Valter Longo, PhD
He met Dr. Valter Longo, PhD at the University of Southern California in 2011. Dr. Longo originally came to the United States from Italy to study to be a rock guitarist and in his college training switched to  biochemistry as a field of study and continued his passion for the causes and determinants of aging and longevity. Dr. Longo is the creator of Prolon® and the Fasting Mimicking Diet manufactured and marketed by L-Nutra. He is the author of  “The Longevity Diet” and currently a professor of gerontology at the University of Southern California and director of their Longevity Institute.

Key Findings of Dwarfism and Reduced IGF1 in Lauren Ecuador
A key crossroads in Dr. Longo’s research on aging was his study of the people of Laron, Ecuador who are missing a gene leading to a defect in the receptor in the liver that binds to growth hormone and produces insulin-like growth factor 1 (IGF1).  This insufficient level of I-GF1 appears to protect them from cancer, diabetes and possibly heart disease and Alzheimer’s disease. So keeping low levels of IGf-1 maybe a key to slowing the aging process and chronic disease development. (Ref. 1, Ref 2)

Fasting Reduces IGF1
Fasting for a few days has been shown to reduce IGF1, Mtorr, PKA and AKT levels which are involved in cancer growth and proliferation. While the long living “Blue Zones” populations practice periodic fasting they don’t routinely do 5 days of fasting as in the 5 day fasting mimicking diet (FMD) concept (Blue Zones interview with Dr. Longo). Through animal and human studies Dr. Longo discovered that fasting for several days (5 days) resulted in a reduction in IGF1 and the stimulation of regenerative processes such as autophagy and stem cell production and differentiation into new tissue.

Fasting Stimulates Stem Cells and Tissue Regeneration
Fasting for a couple of days increases autophagy or “self-eating” which is the body scavenging cell parts or even consuming their own damaged organelles within the cells. While autophagy is always occurring it is increased with fasting for 24-48 hours and with ketosis. There is an increase in circulating stem cells (MSPC – mesenchymal stem/progenitor cells which enhance the formation of organ tissue, muscle and bone) 3-5 days into a fast as damaged cells get deleted and repaired stem cells are increased. With refeeding stem cells go down but remain at a higher level than the baseline prior to the fast. Three to 5 days after the fast, or 10 days from the first day of the fast, maybe half of the stem cells have gone on and differentiated into new tissue. The peak for differentiation of cells is 3-5 days after the end of the fast.

Length and Cycling of FMD
The recommendation for cycling of the FMD diet is 5 days on and 25 days off. While there is benefit to once cycle, the beneficial results are greater and sustained longer with a 3 month cycle of 5 days of fasting. When 3 cycles were accomplished 60%  of the weight loss was maintained without additional changes. The 3 monthly rounds of FMD boosts metabolism.

Humans and Animal Studies
As well as mice and yeast models, human trials have shown reductions in biomarkers for heart disease, cancer, diabetes and autoimmune disease. Mice have been shown to live 13% longer, neural tissue regeneration and enhanced immune function.

FMD and Cancer
Fasting states slow the growth of normal cells dramatically while cancer cells are unaffected by fasting and continue to divide rapidly. The chemotherapy then results in less side effects or damage to normal cells while “killing” the unaffected and rapidly growing cancer cells. Fasting may allow for longer and more potent chemotherapy to be given which also may improve cancer outcome. The fasting has to continue through the administration of the chemotherapy and is started several days before the chemotherapy. There is currently a large human trial now with the FMD and cancer in several cancer centers. Though this is not proven Dr. Kelley believes that fasting boosts the ability of the immune system to “pursue” cancer cells.

FMD and Autoimmunity
Mouse models have shown a dramatic response to fasting with slowing and stopping the progression of multiple sclerosis in more than 80% of animals and a cure in 18-20% of the animals with regeneration of the damaged nerves and myelin regeneration.

There are soups, whole grains, kale crackers olives, nuts, some nut rich food bars (and small chocolate containing bar) packaged to deliver 1100  calories the first day and 800 calories the next four days which can be eaten in a breakfast, lunch, dinner and snack format. Glycerin is provided starting on day 2 through 5 to add to your herbal teas to help prevent muscle loss along with free fatty acids provided from the healthy plant fats.

About half of the people feel that it is easy (for Dr. Kelley it is not easy) while others have discomforting symptoms which usually clear by day 4 or 5. There are lesser percentages of sugars, carbohydrates and protein compared to the healthy fats but enough to prevent muscle loss but still trigger the beneficial effects of fasting mentioned above. Some patients get into mild ketosis some don’t in this 5 day period. There is a refeeding day on day 6. Start with liquids, soft foods, but don’t eat a heavy meat based meal or a high sugary meal.

Three cycles is strongly recommended to have sustained results and then as needed for continued health maintenance and rejuvenation. Chronic conditions may require a monthly fasting regimen until symptom relief and optimal health is achieved.

"Fast Food Genocide"...Is the Massive Consumption of Processed Foods Destroying Our Health and Societal Progress? - An Interview with 6 time New York Times Best Selling Author Joel Fuhrman, MD

Joel Fuhrman, MD, (with Robert Phillips) six-time New York Times best-selling author, writes a different kind of health book in “Fast Food Genocide: How Processed Food is Killing Us and What We Can Do About It”.  He shares not only his vast nutritional knowledge and approach to the prevention and reversal of chronic diseases such as heart disease, diabetes, cancer, obesity, bone loss, neurodegenerative diseases, etc. with his “Nutritarian” approach, but also explains how the high consumption of processed foods affects society’s general progress and functioning from inhibition of learn to increasing the risk to criminal behavior. He notes the high consumption of processed foods puts certain minority groups at a significant disadvantage to live  healthy and prosperous and lives.  Dr. Fuhrman uses the powerful word “Genocide” in his book title to emphasize that eating this way results in the deliberate destruction of an entire race or nation.

Highly processed, “fake” or “Franken Foods,” make up the primary source of U.S. calories and are the primary causes of the chronic disease epidemic in our country. Dr. Fuhrman reviews how we are addicted to these foods do to their high concentration of rapidly absorbed fat, sugar and large amounts of animal protein, probably by design by food manufacturers to increase consumption. When eating more of these “nutrient depleted” foods, food addiction comes the increase in dopamine and other neurochemicals secreted with their consumption, and, by the discomfort from their withdrawal, causing us to eat more of these foods, which compounds one’s health problems and risk. The good news is that the more high-nutrient foods you consume, the less low-nutrient foods you desire.  

Problems caused by this epidemic of Processed Food consumption include:

  • The modern epidemic of heart attacks, stroke, cancer, ADHD, autism, allergies and autoimmune disease
  • Reduced intelligence, learn disabilities, poor school performance and depression
  • Fast food malnutrition resulting in chronic inflammation, weight gain and micronutrient deficiencies that reduce optimal brain functioning
  • Increased addiction to drugs and more violence and crime
  • For African Americans disproportionately higher rates of high blood pressure, fatal stroke and heart attacks, diabetes, severe kidney disease, cancer death and Alzheimer’s disease

The “Great News” is that by applying Dr. Fuhrman’s “Nutritarian” approach of eating  whole, unprocessed foods that have the maximum amount of micronutrients per calorie consumed (Health = Nutrients/Calories) these health problems can not only be prevented but quickly reversed.  Whole food consumption with primarily unprocessed plant-foods is encouraged while using a significantly reduced amount of free-oils in the diet yet getting good fats from whole foods such as nuts, seeds, avocadoes, olives and selective supplementation with omega-3 fatty acid (DHA/EPA) supplementation. Animal foods can be consumed in small amounts as condiments or flavorings in those who just don’t want to give up these foods and excellent health can still be achieved.

Simple concepts and guidelines for Dr. Fuhrman’s “Nutritarian” approach include his recommendations for increased consumption of  G-BOMBS:

  • Greens
  • Beans
  • Onions
  • Mushrooms
  • Berries
  • Seeds

Dr. Fuhrman also discusses his use of water or juice fasting for certain medical conditions and how he personally will practice some extended overnight fasting as a form of health enhancement and to promote longevity. He believes that weight from added lean muscle in the aging process is a distinct health advantage over extra fat weight.

Mold, Mast Cell Activation Syndrome (MCAS), Damaging to Your Brain and Causes Chronic Immune and Inflammation Problems...

Mold can aggravate or cause a wide range of symptoms - including cognitive decline and Alzheimer's. Here, Dr. Carnahan who I have interviewed on the above topic, talks about how mold is a major cause of Mast Cell Activation Syndrome (MCAS), a big term, but really means a chronic aggravation of our immune system which can create chronic inflammation and a wide variety of destructive health problems. Mold toxicity is not difficult to diagnose or treat really, the hardest part is finding and correcting the exposure.... That is the biggest and most difficult problem and most frustrating for me in trying to help patients. I have learned and am learning the hard way by doing work on my own home. I understand it better, but there is no perfect
approach yet.

Alzheimer's Disease Prevention and Reversal...Is It Possible? Yes and Sometimes!

As I was updating my talk on "Alzheimer's Prevention and Reversal. Is It Possible?" for next week (Wednesday, 6:00 pm, 3/14/18, LA BioMed, CDCRC Bldg. 1124 W. Carson, Torrance, CA 310-803-5657 or info@calciumscan.com and Friday, 6:00 p.m. 3/16/18, Health Associates, 3301 Alta Arden, #3 Sacramento, CA - with plant-based potluck there) I came across an interview with Dan Buettner (Bluezones.com) and Mariva Shriver on this very topic of Alzheimer's, longevity and being happy. It is excellent but there is much more to share. (Summary and Video)

The bottom line is Alzheimer's is a lifestyle disease and many of the causes are the same as heart disease, diabetes, etc.. The BIG difference is we have technology to "temporarily" fix the "plumbing and wiring" needed to at least keep people functional with heart disease and diabetes (not the best approaches) even in mid to late stages of these diseases, BUT with Alzheimer's disease, which is a process developing over 20 years or so, if it gets to a certain level, there are ways to approach it, but it is so overwhelming for the family and caretakers, that even if there are valid integrative therapies beyond aggressive lifestyle change like hormones, nutrients, detoxification of environmental toxins, etc. the family/support people do not the energy and resources to "plug those holes".... You must start RIGHT NOW with EXERCISE; a whole-food, unprocessed mostly plant-based diet; SLEEP; community & family; a sense of purpose; continued learning; and avoidance of environmental toxins. The beauty is that everything the prevents heart disease, diabetes, etc. also helps prevent Alzheimer's. BUT start now! Is Cholesterol Good or Bad for Alzheimer's Disease and Cognition?

The brain is 60% fat and 25% of the body's cholesterol is in the brain. But the brain's blood brain barrier blocks the uptake of cholesterol into the brain. So cholesterol is made by the brain to satisfy its enormous needs. HMGCoA reductase inhibitors (statins) block the liver and brain's production of cholesterol. Is that bad? Some say yes! Some aren't all that concerned!

So there is no consensus yet. Though the "leaning" would be to keep cholesterol low by a whole food diet and exercise, and not by the use of statins for brain health.

A lot of my integrative medicine colleagues are against trying to lower cholesterol by any means, especially with statins, because there have been some cases of statin-induced cognitive dysfunction. This could occur not just from the cholesterol lowering ability of the statin, but the statins s ability to reduce mitochondrial energy production in every cell, including the brain. So it might NOT be the cholesterol lowering ability of the statin that causes the problem with statins but the reduction in cellular energy that causes the cognitive decline in the brain cells because statins block CoQ10 production.

So here is my point. You will read/watch in the links below regarding the concerning role of elevated cholesterol in Alzheimer's disease and the vascular disease link to Alzheimer's disease. My take is this. I wouldn't take a statin drug to lower cholesterol, especially if cognitive decline was an issue. But if I had to take a statin I would surely take at least 200-400 mg of ubiquinol (CoQ10) daily with food when taking the statin AND I would get my CoQ10 level measured to make sure it was at least 2.5 μg/ml.

That said, I do believe in lowering cholesterol by a whole food, UNPROCESSED, FIBER RICH, phytonutrient and antioxidant dense, mostly plant based diet which I believe gives the best of all worlds in reducing cholesterol without using a statin, and, has been shown to reverse proven atherosclerosis which affects not only the blood vessels to the heart but everywhere blood vessels go, including the brain! Also elevated cholesterol might increase beta amyloid deposition in the brain a hallmark of Alzheimer's disease.

I do believe in good fats, but they should be obtained mostly, if not all, in whole foods. Eat olives, avocado, raw nuts and seeds, especially flax or chia, and GREENS for brain health. Take a DHA rich omega-3 fatty acid product getting at least 200-300 mg of DHA from algal (algae) oil. They are less contaminated than getting the oil from whole fish and helps maintain fish stocks.

I probably confused you more than I helped..but it is not a clear topic and a work in progress with very strong feelings on both sides.
